| Primary Duties: |
OT: To help patient reach maximum performance within limits of their capabilities by organizing and conducting medically directed occupational therapy programs to prevent disability and restore function following injury, loss of body part; and/or loss of use of body part, and illness.
COTA: To perform and assist in administering occupational therapy treatments under the direction and supervision of the department therapists. |
| Qualifications: |
OT: Graduate with a BS or MS degree in Occupational Therapy from an accredited university as well as being licensed in the state of Illinois. Two to three years’ previous experience as an occupational therapist preferred. Current C.P.R. certificate required.
COTA: Graduate of an accredited occupational therapist assistant program receiving an associate’s degree with current licensure in the State of Illinois. Current C.P.R. certification required. |

| Primary Duties: |
Aids patients and their families with personal and environmental difficulties which predispose illness or interfere with obtaining maximum benefits from medical care; serves as a resource person to assist hospital staff and community members in meeting psycho-social needs. Bachelors Degree required with registry certification or licensure. Assists in the training of staff and volunteer related to psychosocial issues, Hospice philosophy, and stages of death and dying. Qualty Improvement must include clinical record review and patient satisfaction survey. May include effectiveness of pain management, customer/agency/Doctor satisfaction surveys and any other federal/state mandates pertaining to QI activities.

| Qualifications: |
Bachelors Degree in social work by an accredited college required with registry certification or licensure. Masters Degree preferred. Previous experience in medical social service field preferred and knowledge of community resources essential. Understands Hospice philosophy, issues of death/dying. Experience in home health care and/or hospice preferred. Valid Illinois State Driver’s License & vehicle insurance.
