St. Margaret's Patient Portal is Now Live

This online tool gives you the flexibility to access your health information and other resources on your time and between visits. The portal will be available over the internet, which means that you can use it from virtually anywhere. You can also use it to access information for family members and individuals for whom you provide care if given permission.

Rest assured, it is completely secure, so you can be confident that your private information is protected. Only you – or an authorized family member – can access your information.

As a patient of St. Margaret’s, enrolling in the MyCare Patient Portal will allow you to:

Securely message your medical provider’s staff
View appointments
Review lab results
View other reports
Update personal information
Request prescription renewals
View history & discharge information

By reading and signing the Portal Consent, you are agreeing that you have been informed of the Terms and Conditions of the MyCare Patient Portal Program and agree to these Terms and Conditions.

MyCare Patient Adult Portal Proxy Authorization
Please complete this form if you are an adult patient at least 18 years of age and want to give another adult access or grant proxy access to your MyCare Patient Portal Account.

MyCare Patient Portal Parent/Guardian Request for Proxy Access-Minor Patient 12-17 Years Old
If you are a Parent/Court Appointed Guardian/Caregiver of the person who is between 12-17 years of age and wish to have online access to portions of his/her record(s), complete the PARENT OR LEGAL GUARDIAN INFORMATION and PATIENT INFORMATION portions of this form.

Revocation of Proxy Authorization
To revoke your MyCare Patient Portal access to a proxy individual. Please send this written revocation to the St. Margaret’s Hospital Medical Records Department. The proxy access will remain in effect until your revocation has been processed.
