Education Requirements:

Masters Degree in social work by an accredited college required with registry certification or licensure. Previous experience in medical social service field preferred and knowledge of community resources essential.  Understands Hospice philosophy, issues of death/dying.  Experience in home health care and/or hospice preferred.  Valid Illinois State Driver’s License & vehicle insurance.
