School & Sports Physicals

We encourage you to call your primary care provider to make an appointment for your child’s school or sports physical.

 

Neither of our walk-in clinics will be doing Kindergarten school physicals or immunizations. Additionally, if your child has a chronic condition, like asthma, they need to see their own physician.

If you’re unable to see your primary care provider, both walk-in locations provide physicals. Visit one of our walk-in clinics, which include Midtown Health Center in Peru or Center for Family Health in Princeton.

Please download and fill out the appropriate form prior to your child's examination.

Forms must be signed by a parent or legal guardian if the child is under the age of 18. Immunization records are required for all school physicals.

No immunizations will be given if the walk-in clinics are used for physical exams. Immunizations can be done through your primary care provider.
