St. Margaret’s Health is offering sports physical clinics at various locations in the community. See dates, times and other specifics below.
Please note: some of these dates and locations are not open to the general public.
Cost: $25.00, Cash/Check ONLY
Please make checks out to “St. Margaret’s Health.” Forms can also be completed ahead of time OR can be provided at the locations of the physicals. To fill out the appropriate forms ahead of time, click here.

Monday, July 25th: 9AM-12PM at JFK Elementary School in Spring Valley (open to public)
Monday, July 25th: 9AM-1PM at Lincoln Elementary School in LaSalle (LaSalle kids only – school & sports physicals)
Tuesday, July 26th: 1PM-4PM at Illinois Valley Community College in Oglesby (open to public)
Wednesday, July 27th: 1PM-4PM at Illinois Valley Community College in Oglesby (IVCC athletes only)
Tuesday, August 2nd: 10AM-1PM at Oglesby Elementary (open to public)
